You must fill out:
    1. File Folder Page 1
    2. Certificate in lieu of oath
    3. Permit
    4. Any applicable technical section that you need (Building, Fire, Electrical or Plumbing)

DO NOT HESITATE TO CALL US IF YOU HAVE ANY QUESTIONS.

The above are all needed to apply for a basic permit.

    5. If updating an existing permit, use the permit update form along with the applicable technical section
    6. Also available are forms possibly needed during the course of apply for your permit, a frame checklist, a chimney certification or a variation request
